What is Senior Care (SC)?
SC services are to help frail older persons to live in their homes and avoid having to move into a nursing home or other institution. SC is a coordination system of community services for the elderly provided jointly by local aging, social services, health and other agencies. SC services start with an evaluation of the older prson's service needs by Geriatric Evaluation Services staff. Then, a case manager helps locate and arrange for the needed services.
What Services Does SC Provide?
SC provides services such as:
- Bathing and Dressing
- Light Housekeeping
- Meal Preparation
- Shopping/Budgeting
- Transportation
Who Can Get Help From SC?
To be eligible for SC services:
- a person must be 65 years of age or older
- a resident of Maryland
- meet eligibility requirements and annual assistance guidelines
- have a medical condition that puts them at risk of having to enter a nursing home or other institution
Persons who meet these eligibility requirements can receive most SC services at little or no cost to them. Some services are available free to those who have limited financial resources.
Who Administers The SC System?
SC is managed locally through a cooperative agreement between Frederick County Departments of Aging, Social Services and Health. SC is a program of the Maryland Department of Aging and is funded by the State of Maryland.
