MENTAL HEALTH CASE MANAGEMENT

Mental Health Case Management (MHCM) is a program of the Frederick County Department of Social Services. The MHCM program provides mental health case management to adults with serious and persistent mental illness and to children and adolescents with serious emotional disturbance. The purpose of this program is to maintain clients at the highest level of functioning and independence through empowerment, increased accessibility to services and family enhancement.

What Services Will MHCM Provide?
Included but not limited to:

  • Assessing needs
  • Assisting clients in accessing entitlements
  • Coordinating services with providers and clients
  • Developing a service plan with the clients and providers
  • Creating links and facilitating ongoing communication among providers and clients
In Addition MHCM will:
  • Complete initial assessment within 15 days after individual has been determined eligible
  • Complete an initial service plan within 10 days of the initial assessment
  • Provide services outlined in the service plan
  • Update plan every 180 days

Who is eligible for MHCM?
Eligible individuals are those requiring intensive, assertive assistance in accessing services that will foster their emotional well being toward greater independence. Individuals referred should meet specific diagnostic criteria, display impaired role functioning over a period of time resulting from mental illness, and have a history of utilizing restrictive services.

Adults: Must voluntarily choose to participate, be 18 years or older, with chronic mental illness, have limited financial resources, and is a resident of Frederick County.

Children and Adolescents: Parent/guardian must voluntarily choose for them to participate, they must be under 18, be diagnosed with or suspected of having a mental disorder, display impaired role function, and be at risk for needing a higher level of care without the delivery of necessary services.

How do I make a referral?

  1. Complete Referral Form for Mental Health Case Management
  2. Include a copy of the Uniform Treatment Plan (UTP) and a social history.
